To Whom It may Concern
 
I am the chairperson of the Crescent Bay District Camporee which will be
held 
April 29-May 1st, 2005 at Schabarum Regional Park in Rowland Heights.
 
As part of our preparation for the Camporee we need to obtain an
insurance waiver for the amount of $2,000,000.
This waiver should include a policy endorsement naming Los Angeles
County Parks and Recreation as an additional insured on the policy.
 
Can you please tell me what information I need to provide you in order
to generate this document?  Without it, we will be unable to secure 
the site for our camporee.  Our need date is April 15, or three weeks
from now.
