Ahoy, maties!

Spring break approaches, and the salty scent of medical forms wafts gently on the breeze.  These are sooooo important... without a complete med form, no Scout can even board the ship to go to camp.  Adult chaperones, this goes for you too!

Every Scout MUST have a "Class 1" medical form, NEW, EVERY year.  These are available online from the Scout council: www.bsa-la.org .  If you have trouble downloading the form, contact me for help.

Every Scout MUST also have a "Class 2" medical form, but these are good for 36 months.  The following Scouts have Class 2 forms from 2005 that are still good for 2006:

(I have a Class 3 for Bobby Safoyan... dunno why... he needs a Class 2.)  If your name does NOT appear on the list above, you NEED a Class 2 form.

Since a Class 2 form requires a doctor's signature, you will probably need to set a medical appointment for your Scout.  I highly recommend that you set this appointment NOW... if you wait until summer, appointments are often much more difficult to set.  The Class 2 form is available on the Scout Council website --- www.bsa-la.org --- so you should download it and bring it with you to the doctor's office.  Call me if you have trouble getting this form.

Adults:  You need a "Class 3" form.  Find it on the website, or call me if you need help.

Everyone:  In addition to the medical forms, you MUST attach a photocopy of your medical insurance card, front and back.  This ensures speedy treatment in the unlikely event that your Scout becomes ill or injured.

You may send the completed medical forms to me at any time between now and camp (August 20).  Please, please get them to me on time... I hate to nag!
