[Emerald Bay] Med Forms and Appointments

All Scouts and Adults going to EB must have a completed Med Form. The CLASS
1 Form must be new and completed within 6 months of attending camp. A CLASS
2 Form is also required for all Scouts. If you have a completed CLASS 2 Form
that is less than 3 years old, you may use that one, otherwise you need to
have it completed by your Scout's doctor prior to going. I have the Med
Forms from last year in my office. You can arrange with me to pick them up.
If you are an Adult over 40 years of age OR a Scout that is doing High
Adventure at EB, you must have your doctor complete a CLASS 3 Med Form
completed within the past 12 months.

 

Scouts need to contact Lora Chen to schedule an appointment with me to
discuss which Merit Badges they will work on at EB. Each appointment lasts
around 20 minutes. Please be prompt and be flexible in your time slot so
that I do not have two hour blocks between appointments. Available
appointment times are:

Thursday             7/17/08                6:30pm - 9pm

Friday                    7/18/08                1pm - 5pm

Thursday             7/31/08                1pm - 5pm

Friday                    8/1/08                   1pm - 5pm
